Ethinyl Estradiol, Norgestrel falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

Here's the short version: your birth control pill (ethinyl estradiol with norgestrel) is partly broken down by a liver enzyme called CYP1A2. In test-tube studies, Danshen seems to slow that enzyme down. If that happened in your body, the estrogen part could build up a little, which in theory might mean more of the usual hormone-type side effects like nausea, breast tenderness, bloating, or headaches.

The good news is this is mostly lab-bench evidence. When Danshen was tested in real people using another CYP1A2 drug, no meaningful effect showed up, so this concern stays theoretical. Still, because Danshen can also affect blood clotting (and estrogen pills already raise clot risk a bit), I'd run it by your pharmacist or doctor before combining them.

Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Your birth control pill (Ovral and similar combos of ethinyl estradiol and norgestrel) is partly handled by a 'pump' protein in your gut and body called p-glycoprotein, which helps move drugs around and out. Danshen seems to slow that pump down a little. In a small study, it raised levels of another medicine that uses this same pump. So in theory, your hormone levels could run a bit higher than usual.

What might that look like? Possibly more of the usual pill side effects, like nausea, breast tenderness, bloating, or headaches. The evidence here is limited and based on a different drug, so this is more of a 'keep an eye on it' situation than a strong warning. Please don't stop either one on your own. Just check with your pharmacist or doctor before pairing them.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Your birth control pill (ethinyl estradiol with norgestrel) is partly broken down by a liver and gut enzyme called CYP3A4. The interesting thing about Danshen is that it can pull in two directions. When taken regularly for a week or two, early research suggests it speeds this enzyme up, which could clear the hormones out of your body faster. In plain terms, that might lower the hormone levels in your blood and, in theory, make the pill a little less reliable at preventing pregnancy.

This is based on limited human data using a different test drug, not on birth control directly, so it is far from certain. Still, since an unplanned pregnancy is a big deal, I would not gamble. Talk with your pharmacist or doctor before combining them, and ask whether a backup method makes sense.
