Why this applies to Pomalidomide

Pomalidomide falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

Here's the gist: your body partly clears pomalidomide using a liver enzyme called CYP1A2. Danshen, in lab studies, can slow that same enzyme down. If that slowing happened in real life, pomalidomide could clear a little more slowly and build up, which might mean stronger effects and more of its usual side effects, like low blood counts, fatigue, infections, or easy bruising and bleeding.

The honest part: this concern comes only from test-tube research, and when scientists tested it in people using another CYP1A2 drug, danshen didn't actually cause the expected change. So this is mostly theoretical. Still, with a cancer medicine this important, and since danshen can also affect bleeding on its own, please check with your oncology team or pharmacist before combining them.

Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the gist for Pomalyst (pomalidomide). One way your body moves this drug in and out of cells is through a transporter called P-glycoprotein, which acts like a little pump. Danshen has been shown to slow that pump down. In a small study, danshen raised blood levels of another pump-dependent drug by roughly a third.

If the same thing happened with pomalidomide, more of it could linger in your system, which may mean stronger side effects like low blood counts, fatigue, or a higher infection risk. This is a moderate, possible concern based on lower-quality evidence, not a sure thing. Since pomalidomide is a serious cancer medicine, please don't add danshen without first checking with your oncologist or pharmacist.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the thing to know about pairing Danshen with Pomalyst (pomalidomide). Your body clears pomalidomide partly through a liver enzyme called CYP3A4. Some research in healthy men found that taking Danshen for about 10 to 14 days speeds up that enzyme, which could push pomalidomide out of your system faster and weaken how well it controls your cancer. Oddly, a single dose did the opposite and raised drug levels, so the effect isn't simple or predictable.

Pomalidomide is not a prodrug, so faster clearance generally means less benefit. Because this is a serious cancer medicine where steady levels matter, please don't add Danshen without checking with your oncologist or pharmacist first.

What Is Pomalidomide Used For?

Brand name: Pomalyst

Pomalidomide capsules are used in adults with multiple myeloma (a blood cancer affecting plasma cells) who have already tried at least two prior treatments — including lenalidomide and a proteasome inhibitor — and whose disease has continued to progress. It is given in combination with dexamethasone for this use.

Pomalidomide capsules are also used to treat Kaposi sarcoma (KS) — a cancer that can cause lesions on the skin and in internal organs — both in adults with AIDS-related KS after antiretroviral therapy has stopped working, and in adults with KS who are HIV-negative. The KS indication was approved based on response rate; ongoing studies are confirming long-term benefit.
