Loperamide, Simethicone falls under P-GLYCOPROTEIN SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels of drugs cleared by p-glycoprotein.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the gist: Imodium Advanced combines loperamide (which slows down your gut to stop diarrhea) with simethicone (a simple anti-gas ingredient). Loperamide leans on a little cellular pump called p-glycoprotein to help keep it moving out of your body, and one small study suggests Danshen can slow that pump down. If it does, a bit more loperamide could stick around than usual.

What might that look like? Possibly more constipation, cramping, bloating, or drowsiness. This is based on lower-quality evidence, so it's a possible concern, not a sure thing. Occasional, label-dose use is usually low risk, but loperamide at high doses can stress the heart. Before pairing them regularly, please check with your pharmacist or doctor.
