Pomalidomide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group European Barberry interacts with.

Theoretically, European barberry might increase the levels and clinical effects of CYP3A4 substrates.
There is very preliminary evidence suggesting that berberine, a constituent of European barberry, might inhibit the CYP3A4 enzyme. Theoretically, European barberry might have a similar effect.

Here's the concern with combining barberry and Pomalidomide (Pomalyst). Your body relies partly on an enzyme called CYP3A4 to break down and clear pomalidomide. Early lab research hints that berberine, the main active compound in barberry, may slow that enzyme down. If it does, pomalidomide could clear more slowly and build up in your blood, which may mean stronger effects and a higher chance of side effects like low blood counts, fatigue, infections, or an increased risk of blood clots.

I want to be honest that this is theoretical and based on early pharmacology, not strong human studies. Still, pomalidomide is a serious cancer drug with real risks, so I wouldn't gamble. Please run any barberry or berberine supplement by your oncologist or pharmacist before using it.

European barberry is a shrub whose root, bark, and berries contain berberine, a bitter plant alkaloid that has been studied mostly in isolated form. Some early research on berberine is promising for blood sugar and cholesterol, but high-quality evidence for the whole barberry herb is limited. About Pomalidomide

What Is Pomalidomide Used For?

Brand name: Pomalyst

Pomalidomide capsules are used in adults with multiple myeloma (a blood cancer affecting plasma cells) who have already tried at least two prior treatments — including lenalidomide and a proteasome inhibitor — and whose disease has continued to progress. It is given in combination with dexamethasone for this use.

Pomalidomide capsules are also used to treat Kaposi sarcoma (KS) — a cancer that can cause lesions on the skin and in internal organs — both in adults with AIDS-related KS after antiretroviral therapy has stopped working, and in adults with KS who are HIV-negative. The KS indication was approved based on response rate; ongoing studies are confirming long-term benefit.
