Here's the simple version: verapamil already lowers your blood pressure and slows your heart rate. European barberry, mostly because of its berberine content, may have a mild blood-pressure-lowering effect of its own. Stack the two together and the concern is that your pressure could drift a little too low. That might show up as dizziness, lightheadedness when you stand up, unusual tiredness, or a slower-than-usual heartbeat.

I want to be honest about the strength of this, though. It's mostly theoretical, based on how barberry behaves in lab and animal studies, not on strong human proof. So it's worth being aware of, not panicking over. If you'd like to try barberry, check with your pharmacist or doctor first, and keep an eye on your blood pressure readings.

Verapamil also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, European barberry might increase the levels and clinical effects of CYP3A4 substrates.
There is very preliminary evidence suggesting that berberine, a constituent of European barberry, might inhibit the CYP3A4 enzyme. Theoretically, European barberry might have a similar effect.

Here's the simple version: your body uses an enzyme called CYP3A4 to break down verapamil. The berberine in European barberry may slow that enzyme down. If it does, verapamil could hang around longer and build up in your system, which is the opposite of what you'd want with a blood pressure and heart-rhythm medicine.

In real life, that could mean too much verapamil effect: dizziness, lightheadedness, very low blood pressure, a slow heartbeat, swollen ankles, or constipation. Now, the honest caveat: this is theoretical, based on lab thinking and not solid human studies, so it may not amount to much for most people. Still, since verapamil affects your heart rate, I'd run barberry past your pharmacist or doctor before combining them.

European barberry is a shrub whose root, bark, and berries contain berberine, a bitter plant alkaloid that has been studied mostly in isolated form. Some early research on berberine is promising for blood sugar and cholesterol, but high-quality evidence for the whole barberry herb is limited. Talk with your pharmacist or doctor before using it, and avoid it during pregnancy and breastfeeding.

About Verapamil

What Is Verapamil Used For?

Brand names: Isoptin SR, Verelan, Isoptin, Calan SR, Verelan SR, Calan, Covera HS, Verap, Veramil

Verapamil tablets and film-coated tablets are used to treat angina — including vasospastic (Prinzmetal's) angina, unstable angina, and chronic stable angina — as well as certain abnormal heart rhythms (arrhythmias) and high blood pressure (essential hypertension). For arrhythmias, regular-release tablets are used alongside digoxin to control heart rate in atrial flutter or atrial fibrillation, and to help prevent episodes of paroxysmal supraventricular tachycardia (SVT), a type of rapid heartbeat.

Extended-release and sustained-release oral forms (tablets and capsules) are indicated specifically for high blood pressure. Verapamil hydrochloride injection is used in hospital settings for rapid conversion of SVT to normal rhythm and for temporary control of fast heart rates in atrial flutter or atrial fibrillation.
