CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Irinotecan Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the wrinkle with Danshen and irinotecan (Onivyde). Your liver and gut use an enzyme called CYP3A4 to help process irinotecan into its active, cancer-fighting form. The research on Danshen is a bit of a two-way street: taking it daily for a couple weeks seems to speed up CYP3A4, while a single dose may briefly slow it down. That makes the net effect on your chemo hard to predict.
Because irinotecan is a powerful cancer drug with a narrow safety margin, even modest shifts matter. Too much active drug can worsen its known side effects like severe diarrhea and dangerously low blood counts, while faster clearance could weaken its benefit. This is human evidence, but limited, so please don't add Danshen without first checking with your oncology team or pharmacist.

GLUCURONIDATED DRUGS

In vitro research shows that danshen induces the expression of glucuronosyltransferases. However, it also inhibits the activity of glucuronosyltransferases, including various members of the 1A and 2B families. The extent of inhibition of a specific glucuronosyltransferase seems to be dependent on whether or not the danshen is processed via 'sweating'. This type of processing may affect the levels of constituents in danshen that alter glucuronosyltransferase activity. So far, this interaction has not been reported in humans.

Here's the connection worth knowing. Your body clears the active form of irinotecan (called SN-38) partly through a process called glucuronidation, which uses enzymes in the UGT family to tag the drug so it can be removed. Danshen has been shown in lab and animal studies to nudge these same enzymes, sometimes blocking them and sometimes revving them up. If Danshen slows that clearance, SN-38 could linger longer, which might worsen irinotecan's known side effects like severe diarrhea, low blood counts, nausea, and infection risk.
The honest part: this is theoretical and has not actually been reported in people, so I don't want to alarm you. But irinotecan is potent chemo with little room for error, so please clear any Danshen use with your oncology team or pharmacist first.
