CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Ethinyl Estradiol, Norethindrone, Ferrous Fumarate falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for your birth control pill (the ethinyl estradiol and norethindrone combo, with the iron tablets thrown in). In lab dishes, danshen seems to slow down a liver enzyme called CYP1A2, which helps process the estrogen part of your pill. In theory, blocking that enzyme could let estrogen levels run a little higher, which might mean more of the usual estrogen side effects like nausea, breast tenderness, bloating, or headaches.
But here's the important part: this is only seen in test-tube research, not in real people. When scientists tested danshen against an actual CYP1A2 drug in humans, they didn't see the effect. So I wouldn't lose sleep over it. Still, since danshen can also affect blood clotting, it's worth a quick chat with your pharmacist or doctor before combining them.

P-GLYCOPROTEIN SUBSTRATES

Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Your birth control pill (the estrogen and progestin combo, plus the iron in the 'Fe' versions) gets moved around the body partly by a transporter called p-glycoprotein. Danshen appears to slow that transporter down a bit. In one small study it raised the levels of another medicine handled the same way by roughly a third. For your pill, that could nudge hormone levels up slightly, which may mean more of the usual estrogen-type side effects like nausea, breast tenderness, headaches, or bloating.
The evidence here is limited, and Danshen also has its own effects on bleeding and clotting, which matters more with hormones. I wouldn't panic, but please check with your pharmacist or doctor before combining them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the thing with your birth control pill: the estrogen part (ethinyl estradiol) is broken down by an enzyme called CYP3A4 in your gut and liver. The early research suggests that taking Danshen for a couple of weeks can speed that enzyme up. If your body clears the hormone faster, the pill's blood levels can drop, and that matters most for reliable pregnancy prevention. You might also notice more breakthrough bleeding or spotting.
This is based on limited human data (using a different drug, midazolam), so it's a real but not fully proven concern. Interestingly, a single dose did the opposite. I wouldn't change anything on your own. Please check with your pharmacist or doctor, and consider a backup method if you do use Danshen.
