Pralsetinib falls under P-GLYCOPROTEIN SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels of drugs cleared by p-glycoprotein.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the short version: your cancer medicine pralsetinib (Gavreto) is moved through and out of your body partly by a cellular pump called P-glycoprotein. Danshen appears to slow that pump down. In a study of healthy people, danshen raised blood levels of another P-glycoprotein drug by roughly a third. If the same thing happens with pralsetinib, more of it could stay in your system, which may mean more side effects like fatigue, high blood pressure, swelling, mouth sores, or changes in blood counts and liver tests.

This is rated a moderate, possible interaction based on lower-quality evidence, so it's worth attention but not panic. Because pralsetinib treats cancer and has a careful dosing balance, please don't start or stop danshen on your own. Talk with your oncologist or pharmacist first.

Pralsetinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the thing to know about Gavreto (pralsetinib): your body relies on a liver and gut enzyme called CYP3A4 to break it down. Danshen seems to play with that same enzyme. When taken steadily for a couple of weeks, danshen tends to speed up CYP3A4, which could lower your pralsetinib levels and make your cancer treatment work less well. Oddly, a single dose of danshen does the opposite and can raise levels, which might mean more side effects like high blood pressure, fatigue, swelling, or low blood counts.

This is based on early human studies using midazolam, not pralsetinib directly, so it isn't fully proven. Still, with a cancer drug, I wouldn't risk it. Please check with your oncologist or pharmacist before combining the two.

What Is Pralsetinib Used For?

Brand name: Gavreto

Gavreto (pralsetinib) is used to treat adults with metastatic RET fusion-positive non-small cell lung cancer (NSCLC) — a type of lung cancer that has spread and carries a specific genetic alteration detected by an FDA-approved test.

Gavreto is also approved for adults and children aged 12 and older with advanced or metastatic RET fusion-positive thyroid cancer who need systemic (whole-body) treatment and whose cancer no longer responds to radioactive iodine (when radioactive iodine is an appropriate option). This thyroid cancer use is under accelerated approval, meaning continued approval depends on results from confirmatory clinical trials.
