CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Caffeine (OTC drug) falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: your body relies on a liver enzyme called CYP1A2 to clear out caffeine. In test-tube studies, Danshen seems to slow that same enzyme down. If that happened in real life, caffeine could hang around longer and feel stronger than usual, meaning more jitteriness, a racing heart, trouble sleeping, or an upset stomach from a dose that normally feels fine.
The good news is this is mostly theoretical. When researchers tested Danshen with another CYP1A2 drug in actual people, they didn't see a meaningful effect. So I wouldn't lose sleep over it, but if you notice your usual coffee or caffeine pill hitting harder while taking Danshen, ease up and check with your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the honest part first: caffeine is mostly broken down by a liver enzyme called CYP1A2, not CYP3A4, which is the enzyme this Danshen interaction is really about. So the data on file (which looked at a sedative called midazolam) doesn't translate cleanly to your caffeine pill, and any effect is likely to be small.
That said, taking Danshen for a week or two seemed to speed up that enzyme system in study volunteers, while a single dose did the opposite. If there's any overlap with caffeine, you might notice slightly weaker or shorter-lasting effects with regular use, or a bit more jitteriness, a racing heart, or trouble sleeping with one-time use. Danshen can also affect blood clotting, so loop in your pharmacist before combining them.
