Danshen might alter the levels of drugs cleared by p-glycoprotein.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the gist for methylprednisolone (Medrol): Danshen may slow down a transporter in your body called p-glycoprotein, which helps move certain drugs out of cells. Methylprednisolone is a substrate of that transporter, so blocking it could let a bit more of the steroid linger in your system than expected.

In plain terms, that could nudge up the steroid's effects and side effects, things like stomach upset, trouble sleeping, mood changes, higher blood sugar, or fluid retention. To be fair, the evidence here is limited (a small study used a different drug, fexofenadine), and the effect was modest. Concentrated Danshen supplements taken daily are the real concern, not a casual tea. Since Danshen can also affect bleeding, please check with your pharmacist or doctor before combining them.

Methylprednisolone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Your body relies on an enzyme called CYP3A4 to break down methylprednisolone (Medrol). Here's the wrinkle with Danshen: when taken steadily for a week or two, it appears to speed that enzyme up, which means your body could clear the steroid faster than usual. In practice, that may make methylprednisolone work less reliably, so the inflammation, swelling, or allergic flare you're treating might not settle the way you'd expect.

One odd twist from the research is that a single dose of Danshen can do the opposite and raise levels briefly. Because the effect is unpredictable and the evidence is still early, I wouldn't panic, but I also wouldn't combine them on your own. Please check with your pharmacist or doctor first.

What Is Methylprednisolone Used For?

Methylprednisolone tablets (Medrol, Methylpred DP) are used to treat a broad set of conditions including endocrine disorders, rheumatic and joint conditions, autoimmune diseases such as systemic lupus erythematosus, severe allergic states, skin diseases, eye inflammation, breathing disorders like asthma, blood and kidney diseases, and certain cancers.

Methylprednisolone acetate injection (Depo-Medrol and others) is used by intramuscular injection when oral therapy isn't feasible, and can also be given directly into joints, soft tissue, or lesions to treat local inflammation from conditions such as rheumatoid arthritis, bursitis, and skin disorders. The injection is not approved for intrathecal (spinal) use.
