Hydroxyzine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the gist: your body partly relies on an enzyme called CYP3A4 to clear hydroxyzine, and Danshen can nudge that enzyme in different directions depending on how long you take it. In one study, taking Danshen for about 10 to 14 days actually sped up this enzyme, which could mean your body clears hydroxyzine a bit faster and you feel less of its effect. A single dose, oddly, did the opposite and briefly slowed clearance, which could make drowsiness or grogginess stronger.

Honestly, this evidence is modest and was measured with a different medication, not hydroxyzine itself, so the real-world impact is uncertain. Hydroxyzine isn't a prodrug, so it doesn't need activating. Still, before combining them, check with your pharmacist or doctor.

What Is Hydroxyzine Used For?

Brand names: Atarax, Vistaril, Ucerax

Hydroxyzine is used to relieve anxiety and tension related to psychoneurosis (anxiety-based mental health conditions) and as a supportive treatment when anxiety is part of another medical condition. Oral forms — including tablets, capsules, and oral solution — are used for anxiety relief and to manage itching (pruritus) caused by allergic conditions such as chronic urticaria (hives), atopic dermatitis, contact dermatitis, and histamine-driven itch. Oral forms are also used as a sedative before and after surgery or general anesthesia.

Hydroxyzine Hydrochloride intramuscular injection is used specifically when a shot is needed — for example, in acute psychiatric or emotional emergencies, acute alcohol withdrawal with anxiety symptoms or delirium tremens, control of nausea and vomiting (except during pregnancy), and as pre- and post-operative or pre- and post-partum supportive medication to reduce narcotic requirements and control nausea.
