Why this applies to Acalabrutinib

Acalabrutinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the gist for Calquence (acalabrutinib). Your body relies on a liver and gut enzyme called CYP3A4 to break this cancer medicine down. Danshen can nudge that enzyme in two different directions, which makes things tricky. Taken steadily for a week or two, danshen seems to speed up CYP3A4, which could lower your acalabrutinib levels and, in theory, make it work less well against your lymphoma or leukemia. Oddly, a single dose may do the opposite and push levels up, raising the odds of side effects like bruising, bleeding, infections, or headache.

This is moderate-strength evidence from small studies, not proof, but with a cancer drug I don't like guessing. Please don't add danshen without first checking with your oncology team or pharmacist.

Danshen might alter the levels of drugs cleared by p-glycoprotein.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the gist for Calquence (acalabrutinib): your body uses a kind of cellular pump called p-glycoprotein to help move this drug along. Danshen appears to slow that pump down a bit. In one small study, it pushed levels of another pump-dependent drug up by roughly 30 to 40 percent. So in theory, taking concentrated Danshen could let a little more acalabrutinib build up in your system.

If that happened, you might notice more of its usual effects, like bruising, bleeding, headaches, fatigue, or a higher infection risk. On top of that, Danshen itself may thin the blood, which adds to the bleeding concern with this cancer medicine. The evidence here is limited, so don't panic, but please check with your oncology team or pharmacist before using any Danshen supplement.

What Is Acalabrutinib Used For?

Brand name: Calquence

Calquence is used to treat adults with mantle cell lymphoma (MCL) — either newly diagnosed (in combination with bendamustine and rituximab, for patients who are not candidates for a stem cell transplant) or previously treated (after at least one prior therapy). It is also used to treat adults with chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL).

Both the capsule and tablet forms of Calquence are approved for all three of these uses. The specific combination regimen and treatment schedule depend on the condition being treated and what other medicines are used alongside it — your care team will map that out for you.
