Disopyramide Phosphate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the thing about disopyramide: it's an active drug already (not a prodrug), and your body clears it partly through an enzyme called CYP3A4. The research on Danshen shows it can push that enzyme in two directions. When taken regularly for a week or two, Danshen seems to speed up CYP3A4, which could lower your disopyramide levels and let your irregular heartbeat slip back. But a single dose appears to do the opposite, raising drug levels, which could mean more side effects like dry mouth, blurry vision, trouble urinating, or even a more serious rhythm problem.

Because disopyramide is a heart rhythm drug with a narrow safety window, even modest swings matter. Please don't start or stop anything on your own. Just check with your pharmacist or doctor first.

What Is Disopyramide Used For?

Brand names: Norpace CR, Norpace

Disopyramide — available as Disopyramide Phosphate, Norpace, and Norpace CR — is indicated for the treatment of documented ventricular arrhythmias, such as sustained ventricular tachycardia (a fast, dangerous heart rhythm), that a physician judges to be life-threatening.

Because this drug can itself cause arrhythmias, it is not recommended for minor or symptom-free arrhythmias. Treatment is started in a hospital setting so the heart can be closely monitored. Antiarrhythmic drugs like disopyramide have not been shown to improve survival in patients with ventricular arrhythmias.
