Amlodipine Besylate falls under ANTIHYPERTENSIVE DRUGS — the group Danshen interacts with.

Theoretically, taking danshen with antihypertensive drugs might increase the risk of hypotension.
Animal research suggests that danshen can produce dose-dependent hypotensive effects. Furthermore, concomitant use with captopril appears to potentiate this effect.

Here's the simple version: amlodipine already lowers your blood pressure by relaxing your blood vessels. Danshen, in animal studies, seems to do something similar and may add to that effect. So if you took the two together, the worry is that your blood pressure could drop a little more than expected, leaving you feeling lightheaded, dizzy, tired, or even causing a faint feeling when you stand up quickly.

I want to be honest about the strength here: this is mostly theoretical, based on lab and animal research, not solid human data. That doesn't mean ignore it, just don't panic. Before you add Danshen, check in with your pharmacist or doctor so they can keep an eye on your numbers. Please don't change your amlodipine on your own.

Amlodipine Besylate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the gist for amlodipine (Norvasc): your liver and gut use an enzyme called CYP3A4 to clear it out. Danshen can nudge that enzyme in either direction. Taken steadily for a week or two, danshen seems to speed up this enzyme, which could lower your amlodipine levels and let your blood pressure or chest pain creep back. But a single dose of danshen appears to do the opposite, briefly raising levels, which might bring more dizziness, flushing, headache, or ankle swelling.

This is based on early human research using a different drug, so the real-world effect on amlodipine isn't certain. Since amlodipine controls blood pressure quietly, you may not notice a shift right away. Please check with your pharmacist or doctor before mixing the two.

Theoretically, taking danshen in combination with amlodipine may decrease the clinical effects of amlodipine.
In animal research, taking danshen orally in combination with amlodipine reduced blood levels of amlodipine by about 52%. This may have been due to induction of cytochrome P450 3A4 (CYP3A4) by danshen, which has been demonstrated in vitro. So far, this interaction has not been reported in humans.

Here's the gist with Danshen and your amlodipine. Amlodipine relies on a liver enzyme called CYP3A4 to be cleared from your body. The concern is that Danshen may speed up that enzyme, which could push amlodipine out of your system faster and leave less of it working. In animal studies, blood levels of amlodipine dropped by roughly half when Danshen was added.

If that happened to you, your blood pressure might creep back up or your chest pain (angina) could be less controlled. The honest part: this is based on lab and animal work only, with no reports in people yet, so it's more of a theoretical heads-up than a proven problem. Before adding Danshen, check with your pharmacist or doctor so they can keep an eye on your blood pressure.

What Is Amlodipine Used For?

Brand name: Norvasc

Amlodipine treats high blood pressure (hypertension) in adults and children aged 6–17 years. Lowering blood pressure reduces the risk of strokes and heart attacks. Sdamlo (oral solution) is specifically indicated for hypertension in adults and pediatric patients 6 years of age and older.

Amlodipine also treats coronary artery disease in adults, including chronic stable angina (chest pain with exertion), vasospastic angina (Prinzmetal's or variant angina, caused by coronary artery spasms), and documented coronary artery disease in patients without heart failure or significantly reduced heart pumping function.
