Hydroxyzine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group European Barberry interacts with.

Theoretically, European barberry might increase the levels and clinical effects of CYP3A4 substrates.
There is very preliminary evidence suggesting that berberine, a constituent of European barberry, might inhibit the CYP3A4 enzyme. Theoretically, European barberry might have a similar effect.

Here's the basic idea with this pairing. Your body uses a liver enzyme called CYP3A4 to help clear hydroxyzine. Berberine, the main active compound in European barberry, may slow that enzyme down a bit. If it does, hydroxyzine could hang around longer and feel stronger than usual.

In real terms, that might mean more drowsiness, dry mouth, dizziness, or grogginess than you'd normally get. The good news is that this is only a theoretical concern based on how berberine behaves in the lab, not from real patient reports, so it may not amount to much. Still, if you plan to combine them, run it past your pharmacist or doctor first, especially since both can make you sleepy.

Here's the gist with European barberry and hydroxyzine. Hydroxyzine is an older antihistamine that already has mild anticholinergic activity, which is why it can leave you feeling drowsy, dry-mouthed, a little foggy, or even constipated. Lab (test tube) studies hint that barberry may have some anticholinergic-like properties of its own, so the worry is that the two could add up and nudge those effects a bit higher.

I'll be honest though: this is a theoretical concern based on pharmacology and cell studies, not strong human data. Hydroxyzine is not a prodrug, so nothing here changes how it's activated. Tiny culinary amounts of barberry likely don't matter, but concentrated berberine supplements are a different story. If you take both, mention it to your pharmacist or doctor first.

About Hydroxyzine

What Is Hydroxyzine Used For?

Brand names: Atarax, Vistaril, Ucerax

Hydroxyzine is used to relieve anxiety and tension related to psychoneurosis (anxiety-based mental health conditions) and as a supportive treatment when anxiety is part of another medical condition. Oral forms — including tablets, capsules, and oral solution — are used for anxiety relief and to manage itching (pruritus) caused by allergic conditions such as chronic urticaria (hives), atopic dermatitis, contact dermatitis, and histamine-driven itch. Oral forms are also used as a sedative before and after surgery or general anesthesia.

Hydroxyzine Hydrochloride intramuscular injection is used specifically when a shot is needed — for example, in acute psychiatric or emotional emergencies, acute alcohol withdrawal with anxiety symptoms or delirium tremens, control of nausea and vomiting (except during pregnancy), and as pre- and post-operative or pre- and post-partum supportive medication to reduce narcotic requirements and control nausea.
