Why this applies to Dronabinol

Dronabinol falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP2C9.
In vitro research shows that various constituents of danshen inhibit the activity of CYP2C9. So far, this interaction has not been reported in humans.

Here's the gist for these two together. Your body relies on a liver enzyme called CYP2C9 to help clear dronabinol (Marinol, Syndros) out of your system. In lab dish studies, compounds in Danshen appear to slow that same enzyme down. If that were to happen in a real person, dronabinol could hang around longer and feel stronger than expected.

In practice, that might mean more of the usual dronabinol effects: dizziness, drowsiness, a feeling of being unusually high or out of it, a fast heartbeat, or feeling lightheaded when you stand up. Now the honest part: this interaction has only been seen in test-tube research, never reported in actual patients, so it stays theoretical. Still, it's worth a quick chat with your pharmacist or doctor before pairing them.

Dronabinol also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the gist: your body uses a liver and gut enzyme called CYP3A4 to break down dronabinol. Danshen seems to nudge that enzyme in different directions depending on how you take it. Taken steadily for a week or two, the research (done with a different test drug, midazolam) suggests danshen speeds that enzyme up, which could clear dronabinol faster and make it work less well, so you might notice more nausea or less appetite improvement than expected. A single dose of danshen actually did the opposite and raised drug levels, which could mean more dizziness, drowsiness, or that "high" feeling.

This is a moderate, possible interaction based on early human studies, not solid proof for dronabinol itself. Please check with your pharmacist or doctor before combining them, and don't change anything on your own.

What Is Dronabinol Used For?

Brand names: Marinol, Syndros

Dronabinol — available as capsules (Marinol, Dronabinol) and as an oral solution (Syndros, Dronabinol oral solution) — is approved for two conditions in adults. First, it treats anorexia (severely reduced appetite) associated with weight loss in patients with AIDS. Second, it controls nausea and vomiting caused by cancer chemotherapy in patients who have not gotten enough relief from standard anti-nausea treatments.

Both uses apply to all available formulations, but dosing timing and meal instructions differ between the capsule and oral solution products. Always follow the specific instructions for the product you have been prescribed.
