Elagolix Sodium falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the connection: your body uses an enzyme called CYP3A4 to break down Orilissa (elagolix), and Danshen can change how busy that enzyme is. The tricky part is that Danshen seems to work both ways. After taking it for about 10 to 14 days, it speeds up CYP3A4, which could clear elagolix out faster and make it less effective, meaning your endometriosis pain and bleeding might creep back. But a single dose appears to do the opposite, raising drug levels and possibly increasing side effects like hot flashes, headache, or mood changes.

Because this is only early research and the direction can flip, I'd keep it simple: check with your pharmacist or doctor before combining the two. Don't change your Orilissa on your own.

Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the situation with Orilissa (elagolix) and Danshen. Your body uses a kind of cellular pump called P-glycoprotein to move elagolix around and help clear it. Danshen seems to slow that pump down a bit. In a small study, people who took Danshen had higher levels of another pump-dependent drug in their blood. So in theory, pairing Danshen with elagolix could nudge your elagolix levels up.

What might that look like? Possibly more of the usual elagolix effects, like hot flashes, headaches, nausea, mood changes, or trouble sleeping. This is based on limited, lower-quality evidence, so it's a maybe, not a sure thing. Still, please check with your pharmacist or doctor before combining them rather than changing anything on your own.

Orilissa (elagolix) is used to treat moderate to severe pain associated with endometriosis — a condition where tissue similar to the uterine lining grows outside the uterus, causing significant pain.

This includes general pelvic pain as well as pain during sex (dyspareunia). How long you can take it depends on your dose and your overall health, so your prescriber will tailor the treatment duration to you.
