Dronabinol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Grape interacts with.

It is unclear if grape seed extract inhibits or induces CYP3A4; research is conflicting.
In vitro evidence suggests that grape seed extract might inhibit CYP3A4 enzymes. However, evidence from animal research shows that grape seed extract may induce CYP3A4 in the liver. So far, these interactions have not been reported in humans.

Your body relies on a liver enzyme called CYP3A4 to clear dronabinol (Marinol, Syndros). Grape seed extract may tap into this same enzyme, but here's the honest truth: the lab and animal studies pull in opposite directions. Some test-tube work hints that it might slow CYP3A4 (which could let dronabinol build up, leading to more drowsiness, dizziness, a faster heartbeat, or feeling "high"), while animal data suggests it might speed up the enzyme (which could weaken dronabinol's help with nausea or appetite).

So far this hasn't actually shown up in people, and eating grapes as food is very unlikely to matter. If you take a concentrated grape seed supplement, just run it by your pharmacist or doctor first.

It is unclear if grape juice or grape seed extract inhibits CYP2C9; research is conflicting.
In vitro evidence shows that grape seed extract or grape juice might inhibit CYP2C9 enzymes. However, a small pharmacokinetic study in healthy adults shows that drinking 8 ounces of grape juice once does not affect the clearance of flurbiprofen, a probe-drug for CYP2C9 metabolism. The effects of continued grape juice consumption are unclear.

Here's the gist with grape and dronabinol (Marinol or Syndros). Your liver uses an enzyme called CYP2C9 to help break this medicine down. Lab-dish studies hint that grape seed extract or grape juice might slow that enzyme a little. If it did, dronabinol could linger longer and its effects might feel stronger, things like drowsiness, dizziness, a foggy or spacey feeling, dry mouth, or a faster heartbeat.

The good news: this is mostly test-tube evidence, and a small human study found grape juice didn't change how a similar CYP2C9 drug was cleared. So in real life this is unlikely to cause trouble, especially with normal amounts of grapes in your diet. Still, if you plan to use concentrated grape seed supplements, it's worth a quick chat with your pharmacist or doctor first.

What Is Dronabinol Used For?

Brand names: Marinol, Syndros

Dronabinol — available as capsules (Marinol, Dronabinol) and as an oral solution (Syndros, Dronabinol oral solution) — is approved for two conditions in adults. First, it treats anorexia (severely reduced appetite) associated with weight loss in patients with AIDS. Second, it controls nausea and vomiting caused by cancer chemotherapy in patients who have not gotten enough relief from standard anti-nausea treatments.

Both uses apply to all available formulations, but dosing timing and meal instructions differ between the capsule and oral solution products. Always follow the specific instructions for the product you have been prescribed.
