Entrectinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the gist for Rozlytrek (entrectinib): your body relies on a liver and gut enzyme called CYP3A4 to break it down. Danshen can nudge that enzyme, and the direction depends on how you use it. Taken steadily for a week or two, danshen seems to speed up CYP3A4, which could clear entrectinib faster and lower its levels. For a cancer drug, weaker levels may mean it isn't working as hard as it should against your tumor.

Oddly, a single dose of danshen had the opposite effect in early studies, raising drug levels, which could mean more side effects like dizziness, fatigue, swelling, or heart rhythm changes. The evidence is limited and a bit mixed, so please don't start or stop anything on your own. Run it by your oncologist or pharmacist first.

What Is Entrectinib Used For?

Brand name: Rozlytrek

Rozlytrek (entrectinib) is used to treat adult patients with ROS1-positive metastatic non-small cell lung cancer (NSCLC), as confirmed by an FDA-approved test. It is also approved for adult and pediatric patients older than 1 month of age who have solid tumors — any cancer type — that carry an NTRK gene fusion detected by an FDA-approved test, provided the tumor has no known acquired resistance mutation, is metastatic or cannot be surgically removed without severe harm, and has progressed after prior treatment or has no satisfactory alternative therapy.

The NTRK gene fusion approval in solid tumors was granted under accelerated approval, meaning it is based on tumor response rates. Continued approval may depend on results from confirmatory clinical trials.
