Here's the concern with pairing ashwagandha and entrectinib (Rozlytrek). This isn't about one speeding up or slowing down the other. The issue is that both can be hard on your liver. Entrectinib already lists liver problems as a known side effect, and your oncology team likely checks your liver enzymes with regular blood work during treatment.

Ashwagandha, while usually well tolerated, has been tied to rare but serious cases of liver injury. Stacking two things that can stress the liver could raise that risk and make it harder to tell what's causing a problem. Honestly, this is mostly a theoretical worry based on how each works, not proven harm together. Still, please loop in your oncologist before adding ashwagandha, and watch for yellowing skin, dark urine, or unusual fatigue.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP3A4 substrates.

Here's the concern with pairing these two. Your body clears entrectinib (Rozlytrek) largely through a liver enzyme called CYP3A4. Lab studies hint that ashwagandha may speed that enzyme up, which could push entrectinib out of your system a little faster. In theory, that means lower drug levels and possibly less power to hold your cancer in check, and that's a trade you really don't want with a cancer therapy.

I want to be honest about the strength of this, though. It comes only from test-tube research, not from real patients, so it's more of a flag than a proven problem. Because entrectinib is such an important medicine, please don't add ashwagandha without first running it by your oncology team or pharmacist.

What Is Entrectinib Used For?

Brand name: Rozlytrek

Rozlytrek is used to treat adult patients with ROS1-positive metastatic non-small cell lung cancer (NSCLC) — a type of lung cancer where the ROS1 gene is rearranged — as confirmed by an FDA-approved test.

Rozlytrek is also approved for adult and pediatric patients older than 1 month with solid tumors that have an NTRK gene fusion (a specific genetic alteration), detected by an FDA-approved test, that have spread or cannot be removed without causing serious harm, and have worsened after prior treatment or have no good treatment alternatives. This NTRK indication was granted accelerated approval based on tumor response rates; continued approval may depend on results from confirmatory trials.
