Why this applies to Dronabinol

Dronabinol falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might increase the levels and adverse effects of CYP2C9 substrates.

A small clinical study in healthy volunteers shows that taking quercetin 500 mg twice daily for 10 days prior to taking diclofenac, a CYP2C9 substrate, increases diclofenac plasma levels by 75% and prolongs the half-life by 32.5%. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar), a substrate of CYP2C9. Furthermore, laboratory research shows that quercetin inhibits CYP2C9.

Here's the gist for Dronabinol (Marinol, Syndros): your liver uses an enzyme called CYP2C9 to help break this medicine down. High-dose quercetin supplements can slow that enzyme. When it slows, dronabinol may hang around in your body longer and at higher levels, so its effects can feel stronger.

In real terms, that could mean more dizziness, drowsiness, feeling "high" or confused, a faster heartbeat, or stomach upset. This is mostly a theory based on a small human study with a different drug, so it isn't a sure thing. Food-level quercetin from fruits and veggies is very unlikely to matter; concentrated capsules are the concern. Before pairing the two, check with your pharmacist or doctor.

Dronabinol also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, concomitant use might alter the effects and adverse effects of CYP3A4 substrates.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the gist: your body clears dronabinol partly through a liver enzyme called CYP3A4, and quercetin can nudge that enzyme. The tricky part is that quercetin seems to pull in two directions. In some lab and small human studies it slows CYP3A4, which could let dronabinol build up and lead to stronger effects like drowsiness, dizziness, a faster heartbeat, confusion, or feeling unusually "high." In one human study, though, quercetin actually sped the enzyme up, which could mean weaker relief from nausea or poor appetite control.

So the honest answer is that the direction isn't certain, and this is mostly theoretical, based on lab and animal data. Food amounts of quercetin are very unlikely to matter. If you're thinking about a concentrated supplement, run it by your pharmacist or doctor first.

What Is Dronabinol Used For?

Brand names: Marinol, Syndros

Dronabinol — available as capsules (Marinol, Dronabinol) and as an oral solution (Syndros, Dronabinol oral solution) — is approved for two conditions in adults. First, it treats anorexia (severely reduced appetite) associated with weight loss in patients with AIDS. Second, it controls nausea and vomiting caused by cancer chemotherapy in patients who have not gotten enough relief from standard anti-nausea treatments.

Both uses apply to all available formulations, but dosing timing and meal instructions differ between the capsule and oral solution products. Always follow the specific instructions for the product you have been prescribed.
