Aliskiren falls under ANTIHYPERTENSIVE DRUGS — the group Danshen interacts with.

Theoretically, taking danshen with antihypertensive drugs might increase the risk of hypotension.
Animal research suggests that danshen can produce dose-dependent hypotensive effects. Furthermore, concomitant use with captopril appears to potentiate this effect.

Here's the basics of pairing Danshen with aliskiren (Tekturna). Aliskiren already relaxes your blood vessels to bring your blood pressure down. The catch is that Danshen, in animal studies, seems to lower blood pressure too. So stacking the two together could push your numbers lower than you want, which may leave you feeling dizzy, lightheaded, tired, or shaky when you stand up too fast.

I want to be honest about the evidence here: this concern comes from lab and animal research, not solid human studies, so it's more of a theoretical caution than a proven problem. Still, it's worth a quick chat with your pharmacist or doctor before adding Danshen, and keeping an eye on your blood pressure if you do.

Aliskiren also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the gist for aliskiren (Tekturna): it's partly handled by a gut enzyme called CYP3A4, and Danshen can nudge that enzyme either way. With regular use over a couple of weeks, Danshen seems to speed that enzyme up, which could lower aliskiren levels and make your blood pressure control a little weaker. A single dose appears to do the opposite, briefly raising levels, which might mean more dizziness or lightheadedness. Aliskiren is not a prodrug, so these are direct effects.

Honestly, the evidence here is early and modest, and most of it comes from a different drug (midazolam). Still, since blood pressure matters, please check with your pharmacist or doctor before pairing the two, and don't change anything on your own.

Aliskiren and Tekturna tablets are both approved to treat high blood pressure (hypertension) in adults and in children who are at least 6 years old and weigh 50 kg (about 110 lbs) or more. Lowering high blood pressure reduces the risk of serious events like strokes and heart attacks.
