Danshen might alter the levels of drugs cleared by p-glycoprotein.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the short version: doxycycline is moved around the body partly by a transporter called P-glycoprotein. Danshen may slow that transporter down a bit. In a small human study, danshen pushed levels of another P-glycoprotein drug up by roughly a third. If the same thing happens with doxycycline, you could end up with slightly more in your system than expected.

Doxycycline isn't a prodrug, so more drug would mean a stronger dose, which could nudge up side effects like nausea, stomach upset, or sun sensitivity. This is based on limited evidence and is only a possible interaction, so don't panic. Just let your pharmacist or doctor know you take danshen so they can keep an eye on things.

What Is Doxycycline Used For?

Doxycycline belongs to the tetracycline family of antibiotics. It's prescribed for many types of bacterial infections, including respiratory tract infections, sexually transmitted infections (STIs), tick-borne illnesses like Rocky Mountain spotted fever, and eye infections. It's also used to help prevent malaria and, in some cases, as a helper treatment for severe acne.

In situations where penicillin-type antibiotics can't be used — due to allergy, for example — doxycycline can step in as an alternative. It should only be used for infections that are proven or strongly suspected to be caused by bacteria.
