Danshen might alter the levels of drugs cleared by p-glycoprotein.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the gist: erythromycin is moved around your body in part by a transporter called P-glycoprotein. Danshen appears to slow that transporter down. In a small study, danshen raised blood levels of another P-glycoprotein drug by roughly a third. So if the same thing happens with erythromycin, you could end up with a bit more of the antibiotic in your system than expected.

With erythromycin, higher levels can mean more of its usual side effects, like nausea, stomach cramps, or diarrhea. Erythromycin can also affect heart rhythm at higher levels, so that's worth keeping in mind. This is based on limited human data, not strong proof. Please don't stop either one on your own. Just check with your pharmacist or doctor before combining them.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the gist: your body clears erythromycin partly through a liver and gut enzyme called CYP3A4, and Danshen can nudge that enzyme. When taken regularly for a week or two, Danshen seems to speed up this enzyme, which could lower your erythromycin levels. In plain terms, the antibiotic might not reach the strength it needs to fully knock out your infection. That matters, because an underdosed antibiotic can mean the infection lingers or comes back.

One twist: a single dose of Danshen appeared to do the opposite and raise levels briefly. The research here is early and done in healthy volunteers, so this isn't a sure thing. Since you're taking erythromycin to clear a real infection, I'd loop in your pharmacist or doctor before mixing the two rather than guessing.

About Erythromycin

What Is Erythromycin Used For?

Brand names: Erythro-S, Erythrocin, Ethril

Erythromycin treats bacterial infections caused by susceptible organisms. Oral and intravenous forms are used for upper and lower respiratory tract infections, skin and soft-tissue infections, pertussis (whooping cough), Legionnaires' disease, sexually transmitted infections (gonorrhea, syphilis, chlamydia), listeriosis, and other conditions. Erythrocin Lactobionate IV and erythromycin lactobionate injection are specifically for patients who cannot take oral medication or who need rapid, high blood levels due to severe infection — IV therapy should be switched to oral as soon as appropriate.

Erythromycin topical solution 2% and Ery 2% Pads (swabs) are indicated only for the topical treatment of acne vulgaris (acne) and are applied directly to the skin — they are not used for internal infections.
