Why this applies to Midazolam

Midazolam falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the interesting twist with Danshen and midazolam. Your body relies on an enzyme called CYP3A4 to clear midazolam, and Danshen can nudge that enzyme in different directions depending on how long you take it. In one study, taking Danshen for 10 to 14 days actually sped up this enzyme in the gut, dropping midazolam levels by anywhere from about 27% to 80%. That could mean the medication works less well, leaving you more awake, anxious, or aware during a procedure than intended.

Oddly, a single dose of Danshen did the opposite and raised midazolam levels, which could deepen drowsiness. Since midazolam is usually given by a healthcare team for procedures, mention any Danshen use to them or your pharmacist beforehand so they can plan around it.

Here's the tricky part with Danshen and midazolam: it can push the interaction in two different directions depending on timing. Midazolam is broken down by an enzyme called CYP3A4, both in your gut and liver. When Danshen is taken regularly for a week or two, it actually speeds up that enzyme. In a small human study, this lowered midazolam levels quite a bit, which could mean the sedation and calming effect don't work as well as expected.

Oddly, a single dose of Danshen does the opposite and can raise midazolam levels, which might make you more drowsy or slow to wake up. Since midazolam is usually given right before a procedure or surgery, this is worth mentioning to your doctor or anesthesia team beforehand. Please don't adjust anything on your own, just let them know you take Danshen.

About Midazolam

What Is Midazolam Used For?

Brand names: Versed, Seizalam, Nayzilam

Midazolam is used in several different forms depending on the setting. Injectable midazolam (given by IV or IM shot) is used before and during surgery or procedures — such as endoscopy, cardiac catheterization, or bronchoscopy — to cause sedation, reduce anxiety, and prevent memory of the event. It can also be used to start general anesthesia or to keep heavily sedated, ventilated patients comfortable in a critical care unit. Midazolam HCl oral syrup is used specifically in children for sedation and anxiety relief before procedures or anesthesia induction, in a monitored medical setting only.

Nayzilam nasal spray is indicated for a different purpose: the acute treatment of seizure clusters (also called acute repetitive seizures) in people aged 12 and older who have epilepsy. It is used intermittently — not as a daily seizure medicine — when a cluster of seizures breaks through the person's usual pattern.
