Why this applies to Flutamide

Flutamide falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

Here's the gist for your prostate cancer medicine, flutamide. Your liver uses an enzyme called CYP1A2 to help process flutamide. Danshen, in lab and test-tube studies, appears to slow that same enzyme down. In theory, slowing it could let flutamide build up a bit, which might mean stronger effects and a higher chance of side effects like diarrhea, nausea, hot flashes, or liver-related problems (yellowing skin, dark urine, or unusual tiredness).

I want to keep this in perspective, though. This concern comes only from lab work, not from people. When researchers tested another CYP1A2 drug in humans, danshen didn't actually change its levels. Still, danshen can also affect blood clotting, so please check with your oncologist or pharmacist before combining them.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the thing about flutamide: it gets switched on in your body into a more active form, and the enzyme CYP3A4 plays a big role in that. Danshen can affect this same enzyme. When taken regularly for a week or two, danshen seems to speed CYP3A4 up, which could change how much active flutamide your body makes. A single dose appears to do the opposite. Because the timing matters and the effect can go either way, it's hard to predict exactly what would happen for you.

Practically, that uncertainty could mean either weaker cancer-fighting effects or more side effects like liver irritation, diarrhea, or hot flashes. The research here is early and modest, so don't panic, but please check with your oncology team or pharmacist before combining the two.

What Is Flutamide Used For?

Brand names: Eulexin, Euflex

Eulexin (flutamide) capsules are used to treat prostate cancer in men. They are always used together with an LHRH agonist (a hormone-lowering injection), never alone.

Eulexin is indicated for two stages of prostate cancer: locally confined Stage B2-C prostate cancer (used alongside goserelin acetate and radiation therapy, starting eight weeks before radiation begins) and Stage D2 metastatic prostate cancer (cancer that has spread beyond the prostate, taken with an LHRH agonist until the disease progresses).
