Etoposide falls under P-GLYCOPROTEIN SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels of drugs cleared by p-glycoprotein.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the simple version: your body uses a kind of cellular pump called P-glycoprotein to help move etoposide out of cells and clear it from your system. Danshen may slow that pump down. In a small study, danshen raised blood levels of another pump-dependent drug by roughly a third. If the same thing happened with etoposide, more of the chemo could stay in your body than expected.

For a cancer drug like this, that matters. Higher levels could mean rougher side effects, things like low blood counts (raising infection and bleeding risk), nausea, mouth sores, or more fatigue. Danshen may also affect blood clotting on its own. The evidence here is fairly limited, so please don't start or stop anything yourself. Check with your oncology team or pharmacist first.

Etoposide also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the connection worth knowing: your body relies on an enzyme called CYP3A4 to break down etoposide, and Danshen can change how busy that enzyme is. The tricky part is that Danshen seems to work in two directions. When taken regularly for a week or two, it appears to speed up this enzyme, which could clear etoposide faster and possibly make your chemo less effective. A single dose, though, seems to do the opposite and let drug levels climb higher.

Because etoposide is serious cancer treatment where both too little (weaker results) and too much (more low blood counts, nausea, infection risk) really matter, this isn't something to guess about. The evidence here is fairly early, but please loop in your oncologist or pharmacist before using Danshen.

About Etoposide

What Is Etoposide Used For?

Brand names: VePesid, VP16, Etopophos

Etoposide injection (available as Avopef, Etopophos, Toposar, and Etoposide) is used to treat refractory testicular tumors — cancers that have already been treated with surgery, radiation, and other chemotherapy — always in combination with other cancer drugs. It is also used for small cell lung cancer (SCLC) as a first-line treatment, given together with other chemotherapy and, in the case of Avopef, with immunotherapy as well.

Etoposide capsules (VePesid and Etoposide capsules) are approved for small cell lung cancer as first-line treatment in combination with other chemotherapy agents. Capsule forms are not approved for testicular cancer, as the label notes that adequate data on their use in that setting are not available.
