Etoposide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group St. John's Wort interacts with.
St. John's wort induces CYP3A4 enzymes and increases metabolism of CYP3A4 substrates. Clinically significant interactions have been reported with St. John's wort products containing hyperforin 1 mg or more.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the concern with this combination. Etoposide is a chemotherapy drug that your liver clears using an enzyme called CYP3A4. St. John's wort speeds that enzyme up, which means your body can break down and remove etoposide faster than intended. The worry is that your blood levels of the drug could drop, so it may not fight the small cell lung cancer as well as your doctors are counting on.
Etoposide is not a prodrug, so this isn't about boosting side effects. It's about possibly weakening the treatment itself, which is a big deal with cancer care. The evidence here is reasonable, not just theoretical. Please don't start or stop anything on your own. Talk with your oncology team or pharmacist first so your treatment stays on track.

P-GLYCOPROTEIN SUBSTRATES

St. John's wort induces P-glycoprotein. P-glycoprotein is a carrier mechanism responsible for transporting drugs and other substances across cell membranes. When P-glycoprotein is induced in the gastrointestinal (GI) tract, it can prevent the absorption of some medications. In addition, induction of p-glycoprotein can decrease entry of drugs into the central nervous system (CNS) and decrease access to other sites of action.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the concern with pairing St. John's wort and etoposide. St. John's wort revs up a transporter in your gut called P-glycoprotein, which acts like a bouncer that pushes drugs back out before they're fully absorbed. Etoposide rides through that same transporter, so a more active bouncer can mean less of your chemotherapy actually gets into your body.
With a cancer treatment, weaker drug levels are a real worry, because you want the full dose working against those cancer cells. This isn't about new side effects, it's about possibly getting too little benefit. The evidence here is moderate, but given the stakes, please don't take St. John's wort during etoposide treatment without first clearing it with your oncologist or pharmacist.

About Etoposide
What Is Etoposide Used For?
Brand names: VePesid, VP16, Etopophos
Etoposide injection (available as Avopef, Etopophos, Toposar, and Etoposide) is used to treat refractory testicular tumors — cancers that have already been treated with surgery, radiation, and other chemotherapy — always in combination with other cancer drugs. It is also used for small cell lung cancer (SCLC) as a first-line treatment, given together with other chemotherapy and, in the case of Avopef, with immunotherapy as well.
Etoposide capsules (VePesid and Etoposide capsules) are approved for small cell lung cancer as first-line treatment in combination with other chemotherapy agents. Capsule forms are not approved for testicular cancer, as the label notes that adequate data on their use in that setting are not available.
