Janumet combines two diabetes medicines (metformin and sitagliptin) that work to lower your blood sugar. Guarana is basically a strong source of caffeine, and the concern here is mostly theoretical. Caffeine can nudge blood sugar in either direction depending on the person, so in theory it could make your numbers a little harder to predict while you're on Janumet.

Honestly, this is a minor and unlikely interaction, and the evidence behind it is weak. An occasional cup of coffee or tea is very unlikely to throw things off. Where I'd pay more attention is with concentrated guarana energy products or weight-loss supplements taken every day. If you use those, keep an eye on your home glucose readings and mention it to your pharmacist or doctor so they can help you watch for any shifts.

Guarana contains caffeine. Animal research shows that metformin can reduce caffeine metabolism. However, this effect has not been reported in humans.

Here's the interesting twist with Janumet (which combines metformin and sitagliptin) and guarana: the concern isn't really about your diabetes medicine getting stronger or weaker. It's the other way around. Guarana is loaded with caffeine, and some animal studies suggest metformin may slow down how the body clears that caffeine. In theory, the caffeine could hang around longer and hit a little harder.

What that might feel like is more of the usual caffeine stuff: jitteriness, a racing or pounding heart, trouble sleeping, or feeling anxious. Honestly, this has never been confirmed in people, so I wouldn't lose sleep over it (pun intended). If you're sensitive to caffeine or notice these symptoms, just keep your guarana modest and check in with your pharmacist or doctor.
