Why this applies to Phenytoin
Phenytoin falls under HEPATOTOXIC DRUGS — the group Rhubarb interacts with.
Some animal research suggests that anthraquinones in rhubarb might have hepatotoxic effects. Also, rhubarb use has been linked to at least 24 cases of liver injury, although details on the dose of rhubarb and duration of use in these cases is unclear.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the concern with pairing rhubarb root and phenytoin (Dilantin). Phenytoin is already known to be tough on the liver in some people. Rhubarb root contains compounds called anthraquinones, and there's a theory that they may stress the liver too. Stack two things that can each tax the liver, and the worry is the combined load.
I want to be honest about the evidence: this is mostly theoretical, based on animal studies and a handful of unclear case reports, not strong human proof. Eating rhubarb stalks in a pie is not the issue here, concentrated root supplements are. Watch for signs like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and check with your pharmacist or doctor before combining them.

Rhubarb root has a long history of use as a laxative and in traditional Chinese medicine, and its edible stalks are a common food. What Is Phenytoin Used For?
Brand names: Dilantin Kapseals, Dilantin, Di-Phen, Dilantin Infatabs, Phenytek, Dilantin injection
Phenytoin is used to treat and prevent seizures. The specific types covered depend on the formulation. Oral extended-release capsules (including Phenytek), chewable tablets (including Dilantin Infatabs and Phenytoin Infatabs), and oral suspension (including Dilantin-125) are used for tonic-clonic (grand mal) and psychomotor (temporal lobe) seizures; most oral forms also cover prevention and treatment of seizures occurring during or after neurosurgery. Chewable tablets and some capsule formulations also cover complex partial seizures.
Parenteral (injectable) phenytoin sodium is used for generalized tonic-clonic status epilepticus (a prolonged, life-threatening seizure emergency) and for prevention and treatment of seizures during neurosurgery. The IV form can also substitute short-term for oral phenytoin when a patient cannot take it by mouth.
