Penicillin V Potassium falls under NEPHROTOXIC DRUGS — the group Rhubarb interacts with.

Theoretically, long-term use of anthraquinones from rhubarb might increase the risk of nephrotoxicity when used with nephrotoxic drugs.
The anthraquinone constituents of rhubarb have been shown to induce nephrotoxicity in animal research. Additionally, in a case report, a 23-year old female presented with kidney failure after taking 6 tablets of a proprietary slimming agent (found to contain the anthraquinones emodin and aloe-emodin from rhubarb) daily for 6 weeks and then adding diclofenac 25 mg 4 times daily for 2 days. The authors postulate that the anthraquinone constituents of rhubarb contributed to the renal dysfunction, and the addition of diclofenac, a nephrotoxic drug, led to renal failure. Until more is known, advise patients to avoid taking rhubarb if they are taking other potentially nephrotoxic drugs.

Here's the gist: the concern isn't about how your body processes Pen VK. Instead, it's about adding up stress on your kidneys. The anthraquinones in rhubarb root (like emodin and rhein) have been linked to kidney irritation in animal studies and one case report, and penicillins get cleared mostly through the kidneys. So in theory, regularly taking concentrated rhubarb root supplements alongside a drug that leans on your kidneys could add a little extra burden.

That said, I don't want to overstate this. The evidence here is weak (mostly lab and animal data plus a single case involving a much harsher anti-inflammatory). A short course of Pen VK for a sore throat or ear infection is very unlikely to be a problem, and culinary rhubarb stalks in a pie are not the issue. The worry is long-term, high-dose root supplements. If you take rhubarb regularly, just mention it to your pharmacist or doctor so they can check in.

What Is Penicillin V Potassium Used For?

Brand names: V-Cillin K, Veetids, Pen VK, Beepen-VK, Ledercillin, Pen-Vee K

Penicillin V Potassium treats mild to moderately severe bacterial infections caused by penicillin-sensitive bacteria. These include streptococcal infections (such as strep throat and scarlet fever), pneumococcal respiratory infections, mild staphylococcal skin and soft-tissue infections, and fusospirochetosis (Vincent's gingivitis and pharyngitis — a mouth and throat infection).

It is also used as a preventive (prophylactic) therapy: on an ongoing basis to prevent rheumatic fever and chorea from coming back, and — as suggested by the American Heart Association and American Dental Association — to help guard against bacterial endocarditis (infection of the heart valves) in certain patients with heart conditions undergoing dental or upper respiratory tract procedures. It should not be used this way in patients at particularly high risk, such as those with prosthetic heart valves.
