Phenytoin falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Artichoke interacts with.

Theoretically, artichoke might increase serum levels of drugs metabolized by CYP2C19.
In vitro research shows that artichoke leaf extract inhibits CYP2C19 activity. However, this interaction has not been reported in humans.

Here's the gist for phenytoin (Dilantin). In test-tube studies, artichoke leaf extract appears to slow down a liver enzyme called CYP2C19, which is one of the pathways your body uses to clear phenytoin. If that slowdown happened in real life, phenytoin could build up a bit, and that matters because phenytoin is a narrow therapeutic index drug, meaning the line between the right amount and too much is thin. Higher levels can bring on things like dizziness, unsteady walking, slurred speech, double vision, or drowsiness.

Now the reassuring part: this is only lab data, and it has never actually been reported in people. Cooking with artichoke almost certainly won't be an issue. Still, before adding a concentrated artichoke supplement, run it by your pharmacist or doctor, especially since phenytoin levels can be checked with a simple blood test.

What Is Phenytoin Used For?

Brand names: Dilantin Kapseals, Dilantin, Di-Phen, Dilantin Infatabs, Phenytek, Dilantin injection

Phenytoin treats seizures. Oral products are used for generalized tonic-clonic (grand mal) seizures and for psychomotor (temporal lobe) or complex partial seizures. Most oral products are also used to prevent and treat seizures during or after neurosurgery.

Phenytoin Sodium Injection is used for generalized tonic-clonic status epilepticus (a prolonged, dangerous seizure emergency). It is also used for seizures during neurosurgery. The injection should be used only when taking phenytoin by mouth is not possible. Dilantin-125 and Phenytoin oral suspension are listed for tonic-clonic and psychomotor seizures.
