Salmeterol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Polypodium Leucotomos interacts with.

Theoretically, Polypodium leucotomos might increase levels of drugs metabolized by CYP3A4.
In vitro and animal research suggests that Polypodium leucotomos can decrease the metabolism of midazolam by a mechanism possibly related to the inhibition of CYP3A4. However, a small study in humans suggests that taking Polypodium leucotomos with midazolam does not affect the metabolism of midazolam.

Here's the gist: your body clears salmeterol (Serevent) using a liver enzyme called CYP3A4. In a test tube, Polypodium leucotomos looked like it might slow that enzyme down a bit. If that happened in real life, salmeterol could hang around a little longer, which in theory might bump up beta-agonist side effects like a faster or pounding heartbeat, jitteriness, headache, or muscle tremor.

The reassuring part is that this is mostly lab and animal data. When researchers actually tested a similar CYP3A4 drug in people, this fern extract didn't really change anything. So the concern here is small and unlikely. Still, since salmeterol affects your heart and lungs, it's worth a quick word with your pharmacist or doctor before adding this supplement, especially if you notice new palpitations or shakiness.

What Is Salmeterol Oral Inhalation Used For?

Brand name: Serevent

Serevent Diskus (salmeterol inhalation powder) is used for three main purposes: treating asthma in patients aged 4 and older alongside an inhaled corticosteroid (ICS), preventing exercise-induced bronchospasm (EIB) in patients aged 4 and older, and providing long-term maintenance treatment of bronchospasm associated with COPD — including chronic bronchitis and emphysema — in adults.

Important: Serevent Diskus is not a rescue inhaler. It will not relieve a sudden asthma or COPD attack. For asthma, it must always be used together with an inhaled corticosteroid — using it alone for asthma is contraindicated due to a serious risk of asthma-related death.
