Why this applies to Fluticasone

Fluticasone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Polypodium Leucotomos interacts with.

Theoretically, Polypodium leucotomos might increase levels of drugs metabolized by CYP3A4.
In vitro and animal research suggests that Polypodium leucotomos can decrease the metabolism of midazolam by a mechanism possibly related to the inhibition of CYP3A4. However, a small study in humans suggests that taking Polypodium leucotomos with midazolam does not affect the metabolism of midazolam.

Here's the gist: fluticasone (whether you spray it in your nose with Flonase or inhale it with Flovent) is cleared by a liver enzyme called CYP3A4. In a test tube, Polypodium leucotomos looked like it might slow that enzyme down, which could in theory let a bit more fluticasone build up in your body. If that happened, you might see more of the usual steroid effects over time.

That said, the real-world evidence is quite weak. A human study using a similar enzyme test didn't show a meaningful change, so I'd put this in the unlikely and minor bucket. Fluticasone is also barely absorbed when used in the nose or lungs, so the effect is probably small. Still, if you take it regularly, just mention the fern extract to your pharmacist or doctor.
