Verapamil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Polypodium Leucotomos interacts with.

Theoretically, Polypodium leucotomos might increase levels of drugs metabolized by CYP3A4.
In vitro and animal research suggests that Polypodium leucotomos can decrease the metabolism of midazolam by a mechanism possibly related to the inhibition of CYP3A4. However, a small study in humans suggests that taking Polypodium leucotomos with midazolam does not affect the metabolism of midazolam.

Here's the gist for verapamil. Your liver uses an enzyme called CYP3A4 to break verapamil down. The idea on file is that Polypodium leucotomos might slow that enzyme a little, which in theory could let verapamil build up. If that happened, you might notice more of its usual effects, like lower blood pressure, dizziness, a slower heart rate, constipation, or feeling unusually tired.

That said, this is mostly a theoretical concern from lab and animal work. When researchers actually tested the fern extract with a similar CYP3A4 drug in people, it didn't meaningfully change things. So a real-world problem is unlikely. Still, verapamil affects your heart rate and pressure, so it's worth a quick word with your pharmacist or doctor before adding this supplement.

About Verapamil

What Is Verapamil Used For?

Brand names: Isoptin SR, Verelan, Isoptin, Calan SR, Verelan SR, Calan, Covera HS, Verap, Veramil

Verapamil tablets and film-coated tablets are used to treat angina — including vasospastic (Prinzmetal's) angina, unstable angina, and chronic stable angina — as well as certain abnormal heart rhythms (arrhythmias) and high blood pressure (essential hypertension). For arrhythmias, regular-release tablets are used alongside digoxin to control heart rate in atrial flutter or atrial fibrillation, and to help prevent episodes of paroxysmal supraventricular tachycardia (SVT), a type of rapid heartbeat.

Extended-release and sustained-release oral forms (tablets and capsules) are indicated specifically for high blood pressure. Verapamil hydrochloride injection is used in hospital settings for rapid conversion of SVT to normal rhythm and for temporary control of fast heart rates in atrial flutter or atrial fibrillation.
