Rifampin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Polypodium Leucotomos interacts with.

Theoretically, Polypodium leucotomos might increase levels of drugs metabolized by CYP3A4.
In vitro and animal research suggests that Polypodium leucotomos can decrease the metabolism of midazolam by a mechanism possibly related to the inhibition of CYP3A4. However, a small study in humans suggests that taking Polypodium leucotomos with midazolam does not affect the metabolism of midazolam.

Here's the gist for these two together. Rifampin is handled in part by a liver enzyme called CYP3A4, and the theory is that Polypodium leucotomos might gently slow that enzyme down. If that happened, a bit more rifampin could linger in your body, which in theory might mean a slightly higher chance of common rifampin effects like nausea, stomach upset, or the harmless reddish-orange tint it can give to urine, sweat, and tears.

The honest truth is this is mostly theoretical and unlikely to matter. The lab signal showed up with a different drug (midazolam), and a small human study didn't see any real effect. Rifampin is an important TB and meningitis medicine, so don't change anything on your own. Just give your pharmacist or doctor a heads-up before adding this fern extract.

About Rifampin

What Is Rifampin Used For?

Brand names: Rifadin, Rimactane, Rifadin Injection, Rofactor

Rifampin treats all forms of tuberculosis (TB). It is almost always used in combination with other antibiotics — your doctor will build a multi-drug regimen tailored to your infection. Treatment usually runs for at least 6 months total.

Rifampin is also used to clear Neisseria meningitidis bacteria from the nose and throat in people who carry the germ without being sick themselves. It is not used to treat active meningococcal disease. The intravenous (IV) forms — Rifadin IV, Rifadin IV NOVAPLUS, and the injectable form of Rifampin — are specifically indicated when a patient cannot take the drug by mouth.
