Clindamycin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Polypodium Leucotomos interacts with.

Theoretically, Polypodium leucotomos might increase levels of drugs metabolized by CYP3A4.
In vitro and animal research suggests that Polypodium leucotomos can decrease the metabolism of midazolam by a mechanism possibly related to the inhibition of CYP3A4. However, a small study in humans suggests that taking Polypodium leucotomos with midazolam does not affect the metabolism of midazolam.

Here's the basic idea: clindamycin is partly cleared by a liver enzyme called CYP3A4. In test tube and animal studies, Polypodium leucotomos seemed to slow that enzyme down a bit, which could in theory let clindamycin build up slightly. If that happened, you might notice more of its usual side effects, like upset stomach, nausea, or loose stools.

The good news is this is mostly theoretical. When researchers actually tested the fern extract in people using another CYP3A4 drug, it didn't meaningfully change drug levels. So a real-world problem is unlikely, and clindamycin is not a prodrug, meaning its effect shouldn't be weakened. Still, since you're treating an infection, it's worth a quick word with your pharmacist or doctor before combining them.

Clindamycin is prescribed for serious infections caused by certain bacteria — including anaerobic bacteria (bacteria that thrive without oxygen), streptococci, staphylococci, and pneumococci. It's often chosen when penicillin-type antibiotics can't be used, such as in people with a penicillin allergy.

It can treat infections of the lungs, skin and soft tissue, bloodstream, abdomen, and female reproductive organs. Because it carries real risks, doctors reserve it for infections where safer antibiotics aren't appropriate.
