Why this applies to Salmeterol
Salmeterol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Neem interacts with.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Your inhaled salmeterol (Serevent) is cleared from the body mainly by a liver enzyme called CYP3A4. In test-tube studies, neem leaf extract seems to slow that enzyme down. In theory, a slower enzyme means salmeterol could hang around a bit longer and build up, which might nudge up its typical side effects like a fast or pounding heartbeat, shakiness or tremor, jitteriness, headache, or muscle cramps.
The important thing to know: this is only lab-based evidence and has not actually been seen in people, so the real-world risk looks low. Still, don't start, stop, or change anything on your own. If you'd like to use neem, just give your pharmacist or doctor a quick heads-up first.

What Is Salmeterol Oral Inhalation Used For?
Brand name: Serevent
Serevent Diskus (salmeterol inhalation powder) is used for three main purposes: treating asthma in patients aged 4 and older alongside an inhaled corticosteroid (ICS), preventing exercise-induced bronchospasm (EIB) in patients aged 4 and older, and providing long-term maintenance treatment of bronchospasm associated with COPD — including chronic bronchitis and emphysema — in adults.
Important: Serevent Diskus is not a rescue inhaler. It will not relieve a sudden asthma or COPD attack. For asthma, it must always be used together with an inhaled corticosteroid — using it alone for asthma is contraindicated due to a serious risk of asthma-related death.
