Aliskiren falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Neem interacts with.

Theoretically, neem leaf extract might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.

Here's the gist for Tekturna (aliskiren) and neem. Your body uses a liver enzyme called CYP3A4 to help clear aliskiren. In lab dish studies, neem leaf extract appears to slow that enzyme down. If that happened in real life, more aliskiren could linger in your system, which might push its blood-pressure-lowering effect too far. In practice that could mean dizziness, lightheadedness when you stand up, or your potassium creeping higher than it should.

The big caveat: this is theoretical, seen only in test tubes, never reported in people. So please don't panic. Still, since this medicine works on your blood pressure, it's worth a quick chat with your pharmacist or doctor before adding concentrated neem supplements.

Aliskiren and Tekturna tablets are both approved to treat high blood pressure (hypertension) in adults and in children who are at least 6 years old and weigh 50 kg (about 110 lbs) or more. Lowering high blood pressure reduces the risk of serious events like strokes and heart attacks.

Managing blood pressure is usually part of a broader plan that may also include diet changes, exercise, cholesterol control, and other medications. Many people need more than one blood pressure drug to reach their goal.
