Why this applies to Salmeterol

Salmeterol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Coleus interacts with.

Theoretically, taking coleus might decrease serum levels of drugs metabolized by CYP3A4.
In vitro research shows that coleus can activate the nuclear receptor, pregnane X receptor (PXR), which results in increased expression of CYP3A4. Although the clinical significance of this is not known, use caution when considering concomitant use of coleus and other drugs affected by these enzymes.

Here's the basic idea with Coleus and your Salmeterol (Serevent). Your body uses a liver enzyme called CYP3A4 to help clear salmeterol. In lab tests, Coleus appears to nudge your body into making more of that enzyme, which in theory could break salmeterol down faster and leave less of it working in your airways. If that happened, you might notice your inhaler doesn't control the wheezing, chest tightness, or shortness of breath as well as usual.

I'll be honest though: this is only suggested by test-tube research, not real-world cases, so the actual risk is probably small. Salmeterol is a controller medicine you rely on daily, so don't change anything on your own. Just give your pharmacist or doctor a heads-up before adding Coleus.

What Is Salmeterol Oral Inhalation Used For?

Brand name: Serevent

Serevent Diskus (salmeterol inhalation powder) is used for three main purposes: treating asthma in patients aged 4 and older alongside an inhaled corticosteroid (ICS), preventing exercise-induced bronchospasm (EIB) in patients aged 4 and older, and providing long-term maintenance treatment of bronchospasm associated with COPD — including chronic bronchitis and emphysema — in adults.

Important: Serevent Diskus is not a rescue inhaler. It will not relieve a sudden asthma or COPD attack. For asthma, it must always be used together with an inhaled corticosteroid — using it alone for asthma is contraindicated due to a serious risk of asthma-related death.
