Here's the simple version: green tea naturally contains caffeine, and salmeterol (Serevent) is a long-acting beta agonist that already nudges your heart along with opening up your airways. The thought is that stacking caffeine on top of salmeterol could add up and give you a little extra heart stimulation, like a faster heartbeat, palpitations, or feeling jittery or shaky.

I'll be honest with you though, this one is theoretical, based on how the two work rather than real reports of problems. A few cups of green tea aren't likely to cause trouble for most folks. The bigger concern would be concentrated green tea extract pills combined with lots of other caffeine. If you notice a racing heart or tremor, mention it to your pharmacist or doctor.

Salmeterol also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Green tea is unlikely to produce clinically significant changes in the levels and clinical effects of CYP3A4 substrates.
In vitro and in vivo research suggests that green tea can inhibit intestinal CYP3A and induce hepatic CYP3A4 enzymes. However, this effect is unlikely to be clinically significant, as green tea does not appear to affect CYP3A4 activity in humans.

Here's the short version on combining green tea with your Salmeterol (Serevent) inhaler: there's very little to worry about. Salmeterol is processed by a liver enzyme called CYP3A4, and lab studies hint that green tea might nudge this enzyme one way or the other. But in actual people, green tea hasn't shown a meaningful effect on CYP3A4, so your inhaler should keep working just as expected.

This is rated a minor and unlikely interaction, and the evidence comes only from test-tube and animal research. Drinking green tea as a beverage isn't a concern. Concentrated green tea extract pills are stronger, so if you take those, it's worth mentioning to your pharmacist. If you ever notice your breathing isn't as well controlled, check in with your doctor.

What Is Salmeterol Oral Inhalation Used For?

Brand name: Serevent

Serevent Diskus (salmeterol inhalation powder) is used for three main purposes: treating asthma in patients aged 4 and older alongside an inhaled corticosteroid (ICS), preventing exercise-induced bronchospasm (EIB) in patients aged 4 and older, and providing long-term maintenance treatment of bronchospasm associated with COPD — including chronic bronchitis and emphysema — in adults.

Important: Serevent Diskus is not a rescue inhaler. It will not relieve a sudden asthma or COPD attack. For asthma, it must always be used together with an inhaled corticosteroid — using it alone for asthma is contraindicated due to a serious risk of asthma-related death.
