Here's the gist for tamoxifen and green tea: the concern isn't about how your body activates or clears the medicine. It's about the liver itself. Tamoxifen can occasionally stress the liver, and concentrated green tea extract supplements have been tied to rare cases of liver trouble too. Put them together and, in theory, those effects could add up.

I want to be honest about the strength of this, though. It's based on scattered case reports, not solid studies, and it's considered unlikely to happen. Drinking green tea as a beverage is very different from taking high-dose extract capsules. If you do use a supplement, watch for yellowing skin or eyes, dark urine, or unusual fatigue, and check in with your pharmacist or doctor first.

Green tea might increase the levels and adverse effects of P-glycoprotein (P-gp) substrates.
In vitro research and case reports suggest that green tea inhibits drug efflux by P-gp, potentially increasing serum levels of P-gp substrates. Case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking green tea and certain P-gp substrates.

Here's the gist for tamoxifen and green tea. Tamoxifen is moved around your body in part by a little cellular pump called P-glycoprotein. Green tea, in concentrated extract form, may slow that pump down. In theory, that could let a bit more tamoxifen build up in your bloodstream, which might nudge up the usual side effects, things like hot flashes, leg cramps, nausea, or fluid changes.

I'll be honest with you though: this is based on lab studies and a handful of scattered case reports, not strong human research. A normal cup or two of green tea is very unlikely to cause trouble. The bigger question mark is daily concentrated green tea pills. Before adding any green tea supplement, run it by your oncology team or pharmacist first.

Green tea is unlikely to produce clinically significant changes in the levels and clinical effects of CYP3A4 substrates.
In vitro and in vivo research suggests that green tea can inhibit intestinal CYP3A and induce hepatic CYP3A4 enzymes. However, this effect is unlikely to be clinically significant, as green tea does not appear to affect CYP3A4 activity in humans.

Here's the good news for anyone enjoying a cup of green tea while on tamoxifen: this combination is very unlikely to cause problems. Tamoxifen is actually a prodrug, meaning your liver has to convert it into its active forms (mostly through an enzyme called CYP2D6, with some help from CYP3A4) before it can do its job fighting breast cancer.

In test-tube studies, green tea nudged the CYP3A4 enzyme in different directions, but in real people it doesn't seem to meaningfully change how this enzyme works. So your tamoxifen should keep working as expected. This warning rests on lab data only, not human results, so I wouldn't lose sleep over a normal tea habit. Still, if you're considering concentrated green tea extract pills, run that by your pharmacist or oncologist first.

What Is Tamoxifen Used For?

Brand names: Nolvadex, Soltamox

Tamoxifen is used to treat estrogen receptor-positive metastatic breast cancer in both women and men, and as follow-up (adjuvant) therapy after surgery for early-stage breast cancer in women. It also reduces the occurrence of cancer in the opposite breast when used as adjuvant therapy.

In women with ductal carcinoma in situ (DCIS — an early, non-invasive breast change) following surgery and radiation, tamoxifen lowers the risk of developing invasive breast cancer. It is also approved to reduce the risk of breast cancer in women who are at high risk but have not yet been diagnosed. Both Tamoxifen Citrate tablets and Soltamox oral solution share these indications.
