P-GLYCOPROTEIN SUBSTRATES

Saquinavir falls under P-GLYCOPROTEIN SUBSTRATES — the group Green Tea interacts with.
In vitro research and case reports suggest that green tea inhibits drug efflux by P-gp, potentially increasing serum levels of P-gp substrates. Case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking green tea and certain P-gp substrates.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Saquinavir (Invirase): your body uses a tiny pump called P-glycoprotein (P-gp) to push this drug back out of cells. Green tea may slow that pump down. When the pump is blocked, more saquinavir can hang around in your blood, which could nudge up its usual side effects like nausea, diarrhea, stomach upset, headache, or changes in how your body handles fats and sugar.
I want to keep this in perspective, though. The evidence here is mostly lab studies and a few scattered case reports, so it's far from certain. A cup or two of green tea is unlikely to be a big deal for most people, but concentrated green tea extract pills are where I'd be more cautious. Since keeping HIV treatment steady really matters, please run any green tea supplement past your pharmacist or doctor first.

ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP)
2 of 3Saquinavir also falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP).
OATPs are expressed in the small intestine and liver and are responsible for the uptake of drugs and other compounds. Research shows that two of the major catechins found in green tea, epicatechin gallate (ECG) and epigallocatechin gallate (EGCG), inhibit OATPs, specifically OATP1A2, OATP1B1, and OATP2B1. In addition, green tea has been shown to reduce the absorption of some drugs that are OATP substrates, including lisinopril, and celiprolol.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here is the gist for saquinavir (Invirase). Your body uses transporter proteins called OATPs to help shuttle certain drugs from your gut into your blood. The catechins in green tea (especially EGCG) can block those transporters in lab studies. Since saquinavir relies on this kind of uptake, drinking large amounts of green tea or taking concentrated extracts could, in theory, lower how much saquinavir gets absorbed.
Why that matters: saquinavir is an HIV medication, and even small drops in blood levels can let the virus rebound or build resistance, which is a big deal. To be clear, this is based on lab and animal data, not on people, so it is theoretical. A cup or two of green tea is unlikely to be a problem, but please run any green tea supplements past your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
3 of 3Saquinavir also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro and in vivo research suggests that green tea can inhibit intestinal CYP3A and induce hepatic CYP3A4 enzymes. However, this effect is unlikely to be clinically significant, as green tea does not appear to affect CYP3A4 activity in humans.

Here's the short version: if you enjoy a cup or two of green tea while taking saquinavir, you almost certainly have nothing to worry about. Saquinavir is processed by a liver and gut enzyme called CYP3A4, and green tea can tug on that enzyme in both directions in lab studies. It may slow CYP3A4 in the gut (which could nudge drug levels up) and speed it up in the liver (which could nudge them down).
But here's the key point: in actual human studies, green tea doesn't seem to meaningfully change CYP3A4 activity. This is a theoretical, lab-based concern with weak evidence, not something that typically causes real problems. To be safe, concentrated green tea extract pills are a bit different from a regular beverage, so run those by your pharmacist or HIV provider before starting.

Saquinavir is used in combination with ritonavir (Norvir) and other medications to treat human immunodeficiency virus (HIV) infection. Saquinavir is in a class of medications called protease inhibitors. It works by decreasing the amount of HIV in the blood. Although saquinavir does not cure HIV, it may decrease your chance of developing acquired immunodeficiency syndrome (AIDS) and HIV-related illnesses such as serious infections or cancer. Taking these medications along with practicing safer sex and making other lifestyle changes may decrease the risk of transmitting the HIV virus to other people.
