Here's the concern with pairing these two: St. John's wort speeds up a liver enzyme called CYP3A4, and that's the very enzyme your body uses to process saquinavir. When that enzyme runs faster, your saquinavir gets cleared out of your blood more quickly, leaving you with lower levels of the drug.

That matters a lot with an HIV medicine. If saquinavir drops too low, the virus isn't kept in check, and HIV can start to bounce back or even build resistance, which can make future treatment harder. This isn't a theoretical worry either, real studies have shown St. John's wort dropping protease inhibitor levels significantly. Please don't add this herb without first checking with your doctor or pharmacist, who can suggest a safer option for mood support.

St. John's wort decreases the levels and clinical effects of P-glycoprotein substrates.
St. John's wort induces P-glycoprotein. P-glycoprotein is a carrier mechanism responsible for transporting drugs and other substances across cell membranes. When P-glycoprotein is induced in the gastrointestinal (GI) tract, it can prevent the absorption of some medications. In addition, induction of p-glycoprotein can decrease entry of drugs into the central nervous system (CNS) and decrease access to other sites of action.

Here's the concern with pairing St. John's wort and saquinavir (Invirase). This herb speeds up a transporter in your gut called P-glycoprotein, and it also revs up liver enzymes that break saquinavir down. The result is that less of your HIV medicine gets absorbed and stays in your body, so blood levels can drop quite a bit.

That matters a lot here. Saquinavir works by keeping HIV under control, and if levels fall too low, the virus can rebound and even develop resistance, meaning the drug may stop working for good. Because of this, St. John's wort is generally considered something to avoid with HIV protease inhibitors. Please don't start or stop anything on your own, just talk with your pharmacist or HIV doctor first.

In healthy volunteers, St. John's wort can reduce the plasma concentrations of indinavir (Crixivan) by inducing cytochrome P450 3A4 (CYP3A4). This might result in treatment failure and viral resistance. St. John's wort also induces P-glycoprotein, which can result in decreased intracellular protease inhibitor concentrations and increased elimination.

Here's the short version: St. John's wort and saquinavir (Invirase) are a risky pairing, and I'd steer clear of using them together. Saquinavir is an active drug (not a prodrug), so it works as long as enough of it stays in your bloodstream. St. John's wort speeds up the liver enzyme CYP3A4 and a transporter called P-glycoprotein, both of which clear saquinavir out faster.

The real-world worry is that your saquinavir levels drop too low to keep HIV under control. That can mean the virus rebounds and may even become resistant to the medication, which limits your future treatment options. This isn't just a lab theory, it's been shown in people. Please don't change anything on your own, but do talk with your pharmacist or HIV provider before starting any St. John's wort.

Here's the deal with this pairing. We're not talking about St. John's wort changing how saquinavir works in your blood here. Instead, both can make your skin more sensitive to sunlight. St. John's wort contains a compound called hypericin that can do this, and certain drugs used in HIV treatment can add to that effect. Put them together and, in theory, you might sunburn more easily or get a rash with less sun than usual.

I want to keep this in perspective, though. This is a theoretical concern based on how these compounds behave, and research suggests you'd need very high doses of St. John's wort to actually trigger phototoxicity. Bigger picture, St. John's wort has serious, well-known interactions with HIV medicines, so please loop in your pharmacist or doctor before combining them.

What Is Saquinavir Used For?

  • Saquinavir is used in combination with ritonavir (Norvir) and other medications to treat human immunodeficiency virus (HIV) infection. Saquinavir is in a class of medications called protease inhibitors. It works by decreasing the amount of HIV in the blood. Although saquinavir does not cure HIV, it may decrease your chance of developing acquired immunodeficiency syndrome (AIDS) and HIV-related illnesses such as serious infections or cancer. Taking these medications along with practicing safer sex and making other lifestyle changes may decrease the risk of transmitting the HIV virus to other people.
