Salmeterol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Vitamin E interacts with.

Theoretically, vitamin E might induce metabolism of CYP3A4, possibly reducing the levels CYP3A4 substrates.
Vitamin E appears to bind with the nuclear receptor, pregnane X receptor (PXR), which results in increased expression of CYP3A4. Although the clinical significance of this is not known, use caution when considering concomitant use of vitamin E and other drugs affected by these enzymes.

Here's the basic idea with your Serevent (salmeterol) inhaler. Salmeterol is cleared from your body partly by a liver enzyme called CYP3A4. On paper, high-dose vitamin E may gently nudge that enzyme to work a little faster, which could lower salmeterol levels and, in theory, make it a touch less effective at keeping your airways open. Salmeterol is not a prodrug, so this points toward slightly weaker effect, not stronger.

I want to be honest though: this is purely theoretical, based on how the chemicals behave, not real patient reports. Everyday food amounts of vitamin E almost certainly don't matter. If you're taking high-dose supplements and notice more wheezing or shortness of breath, check in with your pharmacist or doctor before changing anything.

What Is Salmeterol Oral Inhalation Used For?

Brand name: Serevent

Serevent Diskus (salmeterol inhalation powder) is used for three main purposes: treating asthma in patients aged 4 and older alongside an inhaled corticosteroid (ICS), preventing exercise-induced bronchospasm (EIB) in patients aged 4 and older, and providing long-term maintenance treatment of bronchospasm associated with COPD — including chronic bronchitis and emphysema — in adults.

Important: Serevent Diskus is not a rescue inhaler. It will not relieve a sudden asthma or COPD attack. For asthma, it must always be used together with an inhaled corticosteroid — using it alone for asthma is contraindicated due to a serious risk of asthma-related death.
