Amprenavir falls under P-GLYCOPROTEIN SUBSTRATES — the group Neem interacts with.
In vitro research shows that neem leaf methanol extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for amprenavir (Agenerase), an HIV protease inhibitor. Your body uses a transporter pump called P-glycoprotein to help move amprenavir around and clear some of it out. In lab and animal studies, neem leaf extract seems to slow that pump down. If the pump works slower, a bit more amprenavir could stay in your body, which in theory might bump up its effects and side effects like nausea, diarrhea, stomach upset, headache, or skin rash.
I want to be honest though: this is only seen in test-tube and animal research, and it has never been reported in actual people. So it's more of a caution than a proven problem. Before adding neem alongside HIV therapy, please check with your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: your body relies on a liver enzyme called CYP3A4 to break down and clear amprenavir, an older HIV protease inhibitor. In lab dish studies, neem leaf extract appears to slow that same enzyme down. If that were to happen in real life, amprenavir could build up a bit higher than expected, which might mean more of its usual side effects like nausea, diarrhea, headache, tingling around the mouth, or rash.
I want to be honest about the strength of this, though. This is theoretical, based only on test-tube work, and has never actually been reported in people taking neem. So please don't panic. Still, since HIV medications matter a lot, just loop in your pharmacist or doctor before adding a concentrated neem supplement.
