Alvimopan falls under P-GLYCOPROTEIN SUBSTRATES — the group Neem interacts with.

Theoretically, neem leaf extract might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that neem leaf methanol extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.

Here's the gist for alvimopan (Entereg). This medicine is one your body moves around with the help of a transporter called P-glycoprotein, which acts like a pump shuttling drugs in and out of cells. In lab dish studies, neem leaf extract seemed to slow that pump down. If that happened in a real person, slightly more alvimopan could stay in the body than expected.

The honest part: this is purely theoretical, based only on test-tube work, and it has never actually been reported in people. Alvimopan is also usually given for a short stretch right around bowel surgery, often in the hospital. Still, since the timing matters, just let your surgical team or pharmacist know about any neem you take so they can keep an eye on things.

What Is Alvimopan Used For?

Brand name: Entereg

Alvimopan capsules are used in hospitals to help the digestive tract recover more quickly after surgery that involves removing part of the bowel (partial bowel resection with primary anastomosis — meaning the remaining ends are reconnected). After this type of surgery, the bowel often temporarily stops working, a condition called postoperative ileus.

This medication is only given during a short hospital stay and is not for long-term or home use.
