Why this applies to Salmeterol

Salmeterol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Fo-ti interacts with.

Theoretically, fo-ti might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research suggests that fo-ti might inhibit CYP3A4. One in vitro study suggests that the degree of CYP3A4 inhibition may depend on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, this evidence conflicts with animal research suggesting that fo-ti does not inhibit CYP3A4. This interaction has not been reported in humans.

Your salmeterol inhaler (Serevent) is cleared from the body in part by a liver enzyme called CYP3A4. The concern here is that fo-ti might slow that enzyme down. If that happened, salmeterol could linger a bit longer and its effects could be amplified. For a long-acting beta agonist, that usually shows up as a faster or pounding heartbeat, jitteriness, shakiness in the hands, headache, or muscle cramps.

The good news: this is very much a theoretical worry. It comes only from lab dish studies, the animal data didn't agree, and it has never been reported in a real person. Still, since fo-ti has also been tied to liver injury, I'd run it past your pharmacist or doctor before combining the two.

What Is Salmeterol Oral Inhalation Used For?

Brand name: Serevent

Serevent Diskus (salmeterol inhalation powder) is used for three main purposes: treating asthma in patients aged 4 and older alongside an inhaled corticosteroid (ICS), preventing exercise-induced bronchospasm (EIB) in patients aged 4 and older, and providing long-term maintenance treatment of bronchospasm associated with COPD — including chronic bronchitis and emphysema — in adults.

Important: Serevent Diskus is not a rescue inhaler. It will not relieve a sudden asthma or COPD attack. For asthma, it must always be used together with an inhaled corticosteroid — using it alone for asthma is contraindicated due to a serious risk of asthma-related death.
