Salmeterol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bulbine Natalensis interacts with.

Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.

Here's the gist with Bulbine natalensis and your salmeterol (Serevent) inhaler. Salmeterol is partly cleared by a liver enzyme called CYP3A4. In lab (test-tube) studies, this supplement seemed to speed up that enzyme. If that holds true in people, your body might break salmeterol down a little faster, which could make it work less well at keeping your airways open. For asthma or COPD, weaker control can mean more wheezing, coughing, chest tightness, or shortness of breath.

I want to be honest though: this is only theoretical and comes from lab data, not human studies, so we really don't know how much it matters in real life. Since salmeterol is a breathing medicine you rely on, I'd run this by your pharmacist or doctor before adding the supplement.

What Is Salmeterol Oral Inhalation Used For?

Brand name: Serevent

Serevent Diskus (salmeterol inhalation powder) is used for three main purposes: treating asthma in patients aged 4 and older alongside an inhaled corticosteroid (ICS), preventing exercise-induced bronchospasm (EIB) in patients aged 4 and older, and providing long-term maintenance treatment of bronchospasm associated with COPD — including chronic bronchitis and emphysema — in adults.

Important: Serevent Diskus is not a rescue inhaler. It will not relieve a sudden asthma or COPD attack. For asthma, it must always be used together with an inhaled corticosteroid — using it alone for asthma is contraindicated due to a serious risk of asthma-related death.
