Salmeterol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Chanca Piedra interacts with.

Theoretically, use of chanca piedra might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.

Your inhaler, salmeterol (Serevent), is cleared from your body mainly by a liver enzyme called CYP3A4. In lab and test-tube studies, chanca piedra appears to slow that same enzyme down. The thinking is that if this enzyme works more slowly, salmeterol could linger and its levels could climb a bit higher than usual.

Salmeterol is not a prodrug, so more drug in your system would mean stronger beta-agonist effects. In practical terms that could mean a racing or pounding heart, jitteriness, shakiness in the hands, or a headache. I want to be honest though: this is only a theoretical concern from lab research and hasn't actually been reported in real people. Still, it's worth a quick chat with your pharmacist or doctor before combining them.

What Is Salmeterol Oral Inhalation Used For?

Brand name: Serevent

Serevent Diskus (salmeterol inhalation powder) is used for three main purposes: treating asthma in patients aged 4 and older alongside an inhaled corticosteroid (ICS), preventing exercise-induced bronchospasm (EIB) in patients aged 4 and older, and providing long-term maintenance treatment of bronchospasm associated with COPD — including chronic bronchitis and emphysema — in adults.

Important: Serevent Diskus is not a rescue inhaler. It will not relieve a sudden asthma or COPD attack. For asthma, it must always be used together with an inhaled corticosteroid — using it alone for asthma is contraindicated due to a serious risk of asthma-related death.
